Progesterone helps the immune system of tolerate foreign DNA (i.e. the fetus.) The endocrine system also begins to produce Relaxin. Relaxin contributes to the softening of pelvic joints to prepare the body for delivery. Relaxin also contributes to the softening of all other joints in the body. Ligament laxity is also caused by the relaxin hormone. Ligament laxity aids in allowing the baby to pass through the pelvis. Can also cause pelvic joints to move unevenly. Ligament laxity can happen as early as the first trimester.
As the baby grows the uterus expands. Women can start to notice a protruding abdomen by the beginning of the second trimester. By the end of the third trimester the uterus will be near the rib cage. As the uterus grows the abdomen protrudes out causing imbalance. Not only does this put strain on the lower back, but also puts added strain and pressure on the internal organs around the uterus.
The pelvis is made up by four bones. These bones are the sacrum, coccyx, and two Ilia (hip bones.) Due to the surge of relaxin these softer bones shift around. Weaker, softer pelvic bones paired with weight gain associated with pregnancy force the pelvis to tilt forward. The spine compensates for this tilt by arching back further. This is done in an effort to maintain balance. A healthy range of weight gain during pregnancy is anywhere from 25- 40lbs. Although healthy, this extra weight contributes to the low back pain many pregnant women suffer.
Pregnancy Discomforts and Labor Difficulties
Throughout pregnancy many women experience discomforts. These discomforts can range from mild headaches, nausea, to more serious back pain and pelvic pain. Mild headaches during pregnancy is common. They can be caused by the surge of hormones. They can also be caused by a pinched nerve in the spine. Nausea during pregnancy is also very common. Nausea can be caused by a surge of hormones or the shifting of the stomach. Back pain and pelvic pain during pregnancy are often experienced where the pelvis meets the spine, and the sacroiliac joint. Pain can be caused by weight gain associated with pregnancy. Weight gain puts added pressure at this intersection. Muscle separation of the abdomen may make back pain worse. Sacroiliac joint pain is another common pain associated with pregnancy. This pain is experienced on either side of hips. Sacroiliac joint pain can sometimes extend down the legs.
Labor difficulties cannot always be prevented. But sometimes there are things that can be done during pregnancy to minimize labor difficulties. More common difficulties are preterm delivery, meconium stained amniotic fluid, use of forceps, and emergency cesarean section. Eleven percent of expectant mothers can go into preterm labor. Preterm is considered 20 – 37 weeks gestation. Preterm labor can be attributed to abnormal uterus and or cervix, and too much or too little weight gain. Meconium stained fluid usually happens when the baby is “under stress.” The baby is considered under stress when blood and oxygen decrease. Use of forceps during delivery is considered when cervix is fully dilated and mother is unable to push baby out. Risks associated with the use of forceps are lower tract and tear wounds, uterine rupture for the mother. The risks to the baby may face are minor facial injuries, temporary weakness in facial muscles, skull fracture, bleeding within skull and seizures. Emergency cesarean sections occur when labor fails to start. They are also performed when labor stalls, the mother or baby are under distress, prolapsed umbilical cord, or uterine rupture. The procedure from start to finish takes about 40 minutes.
